You can add a form for a particular product or for the entire order to collect additional information.

Adding a form for a product

This will add a form for the customer to complete when adding a particular product.

  1. Select [Product Name] under Store Settings --> Products.
  2. Select Add Modifier under General Settings --> Modifiers.
  3. Select the modifier type, such as Drop-Down Selection, Text Entry, or Multiple Select Checkboxes.
  4. Enter or select the various options for each type.
  5. Select Add Modifier.

You can then edit, remove, and sort the modifiers under Current Modifiers to make changes. Make sure to select Save.

Adding a form for the entire order

This will add a form for the customer to complete at the end of the order.

  1. Select User Modifiers under Store Settings.
  2. Select the modifier type, such as Drop-Down Selection, Text Entry, or Multiple Select Checkboxes.
  3. Enter or select the various options for each type.
  4. Select Add Modifier.

You can then edit, remove, and sort the modifiers under Current Modifiers to make changes. Make sure to select Save.
