This form is to be used for any new or renewal contract through the University. This can include:

Maintenance agreements
These are then charged to an IT, departmental, or research index.
Please note that it normally takes approximately two to three weeks to process most contracts from start to finish, please keep this in mind when completing this form. A lot depends on the amount and complexity of the agreement.
